It was an eyebrow-raising decree. Long requested by the profession, the extension of the practice of instrumental abortion to midwives was granted last December, but under conditions that made its implementation almost impossible.
Three doctors present
In fact, this measure, intended to facilitate access to safe abortion, especially in medical deserts, had been subordinated to the mandatory presence of three doctors capable of intervening in the event of a complication (a gynecologist-obstetrician, an anesthesiologist-resuscitator, and a doctor capable of intervening in the event of “arterial embolization”, a serious uterine haemorrhage).
The Order of Midwives deplored a text that produced the opposite effect to that expected, limiting the participation of midwives in instrumental abortion, and therefore, in turn, did not improve their access to patients in any way. The order also noted that the operating conditions imposed by the decree did not exist “not required when doctors perform instrumental abortions or during childbirth, where the risk of bleeding is much higher”.
The government undertakes to rewrite the text

Asked by Franceinfo, Caroline Combot, president of the ONSSF, the main midwives’ union, insisted on the importance of easing the conditions required for the practice of instrumental abortions. In particular to allow its implementation in smaller structures, which cannot necessarily count on the specialist doctors envisaged by the decree at present. “We are always shocked to see that texts can be written by people who know very little about our profession and who try to belittle us and make it difficult for women to access care. »
