Nearly 79 percent of young adults aged 20 to 29 show signs of cardiovascular-kidney-metabolic syndrome, a new study has found. The findings were published in the medical journal Circulation: Population Health and Outcomes.

Researchers found that 79.2 percent of participants had progressed beyond the baseline stage of the condition, despite having an average age of about 23 years. None of the young adults in the study had reached the most severe clinical stage of the disease.

The study analyzed 1,283 participants drawn from the long-term Future of Families and Child Well-Being Study. Investigators determined the specific stage of cardiovascular-kidney-metabolic health for each individual, while using ultrasound imaging of their arteries to assess vascular condition.

Cardiovascular-kidney-metabolic syndrome, commonly known as CKM syndrome, is a medical condition defined by interconnected dysfunction among the heart, blood vessels, kidneys, and metabolic system. First formally defined by the American Heart Association, CKM syndrome reflects how metabolic disorders such as obesity and insulin resistance can impair kidney function and accelerate vascular damage across the body.

Breakdown of syndrome stages

Medical criteria classify CKM syndrome into five distinct stages running from stage 0 through stage 4. Stage 0 represents a complete absence of risk factors, while stage 1 involves an excess or dysfunction of adipose fat tissue. Stage 2 includes metabolic or kidney risk factors, stage 3 indicates subclinical cardiovascular disease or advanced kidney failure, and stage 4 represents full clinical cardiovascular disease.

In the study cohort, 20.7 percent of participants were classified at stage 0 with no identified risk factors. Among those showing signs of the condition, 40.2 percent were at stage 1, 36.2 percent were at stage 2, and 2.8 percent had reached stage 3.

Researchers also evaluated heart and circulatory health using the Life's Essential 8 metric. This measurement scale rates cardiovascular health from 0 to 100 by assessing eight key health indicators and health-related lifestyle behaviors, where higher scores indicate better heart health.

The Life's Essential 8 framework evaluates key factors including diet, physical activity, nicotine exposure, sleep duration, body mass index, blood lipids, blood glucose, and blood pressure. In the study, the average overall score across all participants was 69.2 points.

Cardiovascular health scores varied across the different stages of CKM syndrome. Participants at stage 0 achieved an average score of 77.6 points, while those at stage 1 averaged 70.0 points. Scores averaged 63.8 points for participants at stage 2 and 64.4 points for individuals at stage 3.

Ultrasound evidence of early arterial damage

Ultrasound examinations conducted during the research showed that higher stages of CKM syndrome were directly associated with increased thickness in arterial walls. Thickening of arterial walls serves as a key physical indicator of heightened cardiovascular risk.

Measurement of arterial wall thickness using vascular ultrasound allows medical professionals to detect early structural changes in blood vessels before physical symptoms develop. As arterial walls thicken due to fatty deposits or inflammation, blood vessels become less elastic and narrow over time, increasing the risk of long-term cardiovascular complications.

The study demonstrates that structural changes in human arteries can begin during a person's twenties, long before external symptoms appear. This indicates an early emergence of cardiovascular risks that medical professionals traditionally considered a health issue primary to older populations.

Health experts estimate that these early vascular and metabolic changes can lead to premature heart attacks, strokes, and severe kidney disease at a younger age if left unaddressed.

Research on hypertension and physical activity

In related scientific findings noted alongside the study, researchers from Australia and Singapore found that anti-inflammatory mechanisms play an important role in the development of hypertension. Their research highlights how inflammatory responses influence blood pressure regulation.

Scientists also reported that adding just five extra minutes of physical activity each day can improve health outcomes for individuals living with elevated blood pressure.
