Doctors who condemned the incident in the Journal of Medical Case Reports said the man was taken to the emergency room of Cho Ray Hospital.

The metal nut was driven 5 cm (1.9 in) into his penis and “tightened on the shaft of the penis”.

Doctors noted that this caused the patient to have difficulty urinating and decreased sensation in the genitals.

The man’s foreskin, which had retracted during masturbation, was so swollen that he could not return to its normal position and “strangulated” the tip of his penis.

The patient was connected to an intravenous drip and given painkillers, antibiotics and tranquilizers.

Doctors have noted that the sexual act is sometimes performed even by people with mental illness.

Items such as heavy metal rings and cones, tubes, plastic bottle necks and health bracelets have been reported as cases of penile strangulation.

They warned that “immediate” medical attention is needed when cock-rings become clogged, as they can damage the blood vessels that carry blood between the organs and the heart.

Each case of penile strangulation is treated individually, depending on the patient’s condition. They noted that most patients delayed seeking treatment because of embarrassment.

Thin, non-metallic phallic rings are “usually easy to remove”, but metal objects are “difficult to safely remove” because standard surgical equipment “can’t cut”, the doctors said.

Doctors who treat patients with penile strangulation need to be “creative and resourceful” because not all treatments using prior saws and forceps will work for all patients.

The team noted that using a dental drill is a “rare” method for removing penile strangulation devices, but it is an “excellent option” because it “cuts very smoothly in a short time without significant physical exertion.”
