Levine, a professor who analyzed hormone trends across populations, stated that male testosterone levels are decreasing by more than one percent annually. He described the drop as a consistent trend rather than a statistical error, warning that the reproductive health crisis is already occurring worldwide while humanity pays too little attention to it.

The World Health Organization sets the lower limit of normal sperm concentration at 16 million sperm per millilitre of semen. The agency emphasizes that sperm quality, particularly motility and structural shape, is just as critical as overall count. O'Bryan explained that a high cell count does not guarantee high fertility, noting that an ejaculate containing 100 million sperm is ineffective if the cells cannot swim properly.

Stanislav Lys, a urologist at Yusupov Hospital speaking to Lenta.ru, confirmed that male sperm quality is declining globally, though researchers have not isolated a single primary cause. Lys attributed the decline to environmental pollution, chronic fatigue, life stress, and habits such as smoking, alcohol consumption, and the use of potent substances.

Lys highlighted obesity and type 2 diabetes as major drivers of declining fertility. Excess body weight disrupts hormonal balance and male reproductive function, while increased physical heat in the testicular area further impairs sperm formation. Lys stated that maintaining normal testosterone levels does not require specialized medical treatments. Instead, basic measures such as regular physical activity, adequate sleep, balanced nutrition, stress reduction, and avoiding harmful habits are effective. However, Lys noted that adhering to these recommendations is difficult in large cities, where residents often experience chronic exhaustion and sleep deprivation for years.

Medical experts emphasize that population trends do not imply every man will become infertile within 20 years. Projections made by researchers describe broad demographic patterns rather than individual medical prognoses, and it remains uncertain whether metrics will continue falling at the same pace. Nevertheless, Lys noted that if current trends persist, men will encounter reproductive health disorders with increasing frequency.
Spermograms, which analyze semen composition, offer insights into overall physical health beyond reproductive capacity. O'Bryan pointed out that any abnormal spermogram result can indicate underlying health issues, noting that temporary illnesses like COVID-19 or physical trauma like a broken arm cause measurable drops in sperm counts.
Severe testosterone deficiency impacts multiple organ systems beyond sexual health, affecting the heart, blood vessels, metabolism, and nervous system. Jeff Foster, a specialist in male sexual health, described low testosterone as a catastrophe for physical quality of life, noting that affected men experience frequent illness, severe fatigue, psychological distress, and sexual difficulties.
